I have filled out this form to the best of my ability and I understand that the information given by me will be used to create a personalized custom tattoo. While every effort will be made to incorporate all of the design elements requested I understand that final approval is at the artist's discretion and small changes or omissions may be required in some cases to ensure the best possible outcome of the tattoo.

I understand that any major design changes made without adequate notice may result in the loss of appointment and or deposit at artist's discretion.

I understand that touch ups are up to the artist's discretion within 6 months of receiving the tattoo.I understand that aftercare was given and if proper care was not taken there will be an extra charge .

I understand that a Non-refundable deposit is required for all tattoo appointments booked. The fee will be taken off the cost of the tattoo for the session. If an appointment is not booked within 90 days of the deposit being taken the deposit will be forfeited.

I understand the deposit covers one major edit of the design. If there are more edits I will need to pay an extra design fee that will be added to the total price of the tattoo. Complete design changes may result in reschedule or loss of appointment and deposit at the discretion of the artist.

I understand that if I no-show, or cancel my deposit for any reason will be forfeited.

I understand appointments can be moved with 72 hours notice and the deposit will be transferred. Last minute rescheduling and transfer of deposits to new appointments are to be approved by the artist depending on the reason needed for rescheduling. You may only move your appointment twice without losing the deposit, depending on the reason at the artist’s discretion.

I understand that is the artist is unable to make the appointment due to unseen circumstances (ie: family emergency or sudden illness), the appointment and deposit will be moved to the artist's soonest availability.

I understand that Deposits are non-refundable and non-transferable.

I understand that Gift certificates can not be used for deposits.
